A
medication regimen review (MRR) is a resident-focused, quality assurance
process which involves a systematic evaluation of the resident's complete
medication treatment regimen with the communication of, and the follow-up
on, findings and recommendations. It is a process which facilitates
the application, by Sydney Medication Monitoring Consultants, of specific
skills and knowledge to support and assist other health professionals and
contribute to the care residents receive.
The
MRR is a comprehensive and integrated activity in that it draws on information
from all data sources and all aspects of care relating to residents.
It involves clarification of the indication for use and administration
details of all regular and 'prn' prescription and non-prescription medicines
(incuding nutritional supplements, vitamins, herbal/complementary medicines
and other remedies) being used/taken by the resident.
A medication
chart review (MCR) is an integral part of the MRR, but additional information
sources are used to collate complete drug treatment information. This information
is evaluated in the context of the other resident specific information/data
gathered (in the resident medication profile) to identify actual, suspected
or potential drug-related issues or needs.
The
process results in the generation of a report from Sydney Medication Monitoring
Consultants (as distinct from a care plan) which documents interventions
and/or recommendations intended to overcome the drug-related issues or
needs identified at review. Where they are known at the time of reporting,
the report will also record outcomes resulting from the interventions and
recommendations. Where they are not known, it is expected that the Sydney
Medication Monitoring Consultants will pursue appropriate follow-up activities. |
