- Contribute
to optimising the therapeutic effectiveness and manageability of the resident's
medication regimen.
- Facilitate
a cooperative working relationship between the Sydney Medication Monitoring
Consultants and other members of the healthcare team in order to benefit
the health and wellbeing of residents.
- Provide an information resource of colleagues, residents and other health professionals in relation to medications and medication review process.
Sydney Medication Monitoring Consultants are members of the Australian Association of Consultant Pharmacy. All Services provided by Sydney Medication Monitoring Consultants are funded by the Commonwealth Government of Australia. (Subject to approval). S.M.M.C. 1999/2010..
